1. When you were a child, was it acceptable to discuss death in your family? Did your parents themselves explain death to you? If not, from whom and under what circumstances did you first learn of it?

2. To the best of your recollection, at what age did you have your first direct encounter with the death of a loved one or pet? How did you support yourself or how were you supported through the experience?

3. In retrospect, when dealing with the death of a loved one, is there anything you would have changed if you could have?

4. At this point in your life, is there a specific death that has most impacted you? Whose was it and how did you process your feelings of loss and grief, or the lack of feeling loss or grief?

5. What is the most challenging aspect of death for you to deal with?

6. Would you be comfortable having an intimate conversation with a dying person?

7. Are you afraid of death itself, or more afraid of how you will die?

8. Do you wish you knew the time of your death?

9. Have you completed a Healthcare Directive describing whether you do or don’t want to be placed on life support or receive extraordinary measures? Have you assigned power of attorney to someone you trust will execute your wishes, even if they aren’t a family member?

10. Have you created a will or living trust in the event of your death? Have you discussed with your spouse/partner/children/or in the absence of them a friend, how you wish your remains to be handled? What kind of funeral or memorial you would like?

11. If you want to be cremated, have you indicated where you would like your ashes to be spread or to whom they would be given?

12. Have you ever created your own obituary—mentally or in writing? Do you find the very thought of it unappealing, or interesting?

13. How long do you expect your spouse or partner to wait before they move on with their life after your death? Have you discussed this with him or her?

14. Do you have a “bucket list”? If not, would you consider creating one? If so, have you begun checking off any items?
